Overview

Commonwealth Care Alliance (CCA) is a nonprofit healthcare services organization based in Boston, Massachusetts. Founded in 2003, CCA focuses on delivering integrated care for individuals with complex health needs, serving around 100,000 members, including seniors and those eligible for both Medicare and Medicaid.

CCA employs a unique model called "uncommon care®," emphasizing person-centered care coordination. This model leverages local staff, provider partners, and community organizations to create individualized plans that improve health outcomes while reducing costs. CCA offers diverse health plans and care delivery programs, including Senior Care Options, One Care Dual Demonstration Programs, and specialized hospitalist and palliative care services. In 2024, CCA was acquired by CareSource but continues operating its health plans and clinical programs under the CareSource Family of Brands, maintaining its commitment to high-quality, coordinated care for individuals with significant health and social needs.
